Passport
The Health Care Passport was commissioned by East Kent Hospitals University, part of the NHS Foundation Trust. They required a well designed professional passport for Adults with Learning Difficulties. This passport was to ensure that patient information was available to the hospital team and would ensure that the right information was accessible at all times.
